About 4 years ago I worked for a company that closed its doors. I never got my last 2 weeks pay or my last weeks vacation. Fastforward to today I get a notice of chapter 7 bankrupcy case, meeting of creditors, and deadlines. What does this mean for me? It has a meeting of creditors date.

If the Trustee sends you a notice of possible dividends, you should file a proof of claim. You should start assembling documentation to prove your claim in the meantime.

What if I don't have any old time cards anymore?
IMO- If they sent you a notice, they have you (and maybe other employees who are owed money) as being on the payrol then.
Look for any old pay stubs, maybe old tax returns to show you did work there...anything.
Or try and call them? I do know, these actions can take some time, and if your old company went under, you may not get all
your owed....good luck!

Good luck, we were lucky when it happened to me, we got 100% of my back pay, many only get 10%.

I would try to find my last pay stub (this establishes proof of when you were last paid), then get the place to confirm when your last day of work was (unless you luckily have proof yourself like an email from your boss telling you "ok today's your last day working). I think that by law they'd have to file final payroll tax returns, and when that amount for your tax ID shows the same amount on your last pay stub, yet your last day of working was well after the previously paid pay period, then bam you've got proof.
Proving unpaid vacation will be much more difficult and IMO need to rely on their records?
I was part of something like this once, bit the circumstances were different and I literally had to do nothing. By not responding, I was saying I wanted to be involved. Then i just waited for my check.

Personal affidavit will do. Attend the bankruptcy creditor meeting - and trustee will most likely grand you some kind of lump sum or payment plan.
