Went shooting with a few friends this weekend and heres what went down:
3 rifles were S&W M&P-15 MOE, S&W M&P-15TS, Sig M400.
The guy with the TS would take one, sometimes two shots and experience a failure to feed malfunction. After clearing the round out, the bullet was very much bent (sorry, no pics). Tried a variety of different mags as well as ammo and the same thing continued to happen. Finally the TS and the Sig M400 swapped BCGs and voila, the TS functioned flawlessly. Whats even weirder is that the Sig M400 ran flawless also (with the TS BCG installed). Either way, thought I would share, we're going to contact Smith & Wesson about this because the TS is a very new rifle (less than 200rds).

Any theories or similar experiences?

After shooting the BGC in the Sig did they try it again in the M&P?

Did not. Thats something we should've tried though. Also forgot to mention, the rifles are all factory rifles and not home builds

I would take a good look at the face of the BGC for debris could of been stripping the rounds funny and jamming them into the feed ramps. When you disassembled them it could have dislodged the debris and through for it worked in the SIG.

Factory ammo?

BCG wet with oil or dry?

Did you try different mags?

Ammo was all factory and was brass. The BCG was lubed (MPRO-7). Tried a variety of PMAGS (gen-2) with the same result.

I have an AR that used to do that.. but its a piston upper so I adjusted the gas and it works flawlessly. Over gas issue? Sorry I cannot be more helpful.

"the bullet was very much bent". Was the cartridge case bent or was the bullet (projectile) actually the bent item?

Fail to chamber damage usually is in the neck area... I am assuming that's what he is experiencing. Dent or bent in the middle of the case is usually due to deflector which probably not the case. I am curious to the issue myself.

You can send it back but I would try more tests of my own first before wasting yours(and S&W's) time.

Swap the parts back after a thorough cleaning of all parts and run a few hundred more rounds through it.

I have had many new AR's have issues right out the gate. My resolutions have been anywhere from running the rifle really sopping wet with CLP for 100-200 rounds to swapping a used bolt in it for 200 rounds. After that they have all worked perfectly since.

These have all been DPMS/Rock River/Stag/Spike/etc.

Not to be beating a dead horse [beatdeadhorse]but apparently the projectiles themselves were not actually bent but were just seated crooked in the cases (perhaps like leaning a bit ) ????
i am only making this comment because it really takes a lot to bend an unfired projectile but maybe this shows how little I know about this particular subject.

Did you clean each rifle before you went out and fired it? Recommend always giving it a good bath before firing. I have heard, and seen a lot of malfunctions corrected with a good cleaning.

Unfortunately no pics of the rounds. Yes, rifles were cleaned prior to shooting. As mentioned earlier, possibly a breaking in issue, or some tolerance issues with the TS. As far as the bend... The projectile itself was straight, however it was bent out of the brass neck. Sorry, its difficult to explain w/o a pic
