Where can I get ATF Form 8 5310.11 part II ?

When searching the ATF site, I cannot locate ATF Form 8 5310.11 part II. for C&R license renewal?
I did not receive one in the mail, as expected.

I find:

http://www.gpo.gov/fdsys/pkg/FR-2011-11-08/pdf/2011-28802.pdf- but that is just a notice, not a form

http://www.atf.gov/content/library/firearms-forms - but the form is not listed

http://www.reginfo.gov/public/do/DownloadDocument?documentID=293470&version=2 - but the form is pre-filled out, as an example. I could photoshop out the 'example text' but I am not sure if the form will get rejected if the 1NOV11 dated version is not the latest version.

I attempt to 'order' one from the Order Form page http://www.atf.gov/content/distribution-center-order-form - but the form is not listed as a 'selection'

I will call ATF tomorrow, unless someone here can help me find a PDF version.

According to the ATF, they do not post any 5310.11 forms online, as the renewal form comes partially filled out by the ATF. One can call them and get a duplicate mailed out.

Use (866) 662-2750 (Toll Free) and do not press any "Dial 2 for ..." type buttons . (Even though there is a "Press 1 for Licensing" prompt)
Just wait for an operator and ask for licensing.
