Looking for Custom Name Tape/Embroidery

I know there are several places online that offer such services, but I wanted to check here and see if anyone had this capability:

Colorado Mills mall has a store that does custom embroidery on hats and shirts. They might be able to do those.

Any dry cleaning shop near a military base will make them up for you pretty cheap.
