Maverick Form 1 questions

I am preparing to submit a Form 1 package to register a Short Barrel Shotgun. I have a couple questions.

I need to know what to put in block 4a, Name and Address of Original Manufacturer. The only engraved marking on the shotgun is “Maverick by Mossberg”. Should I put all of that in there or just Maverick or Mossberg?

The other question is what address do I put in the ORI box on the FD-258 Fingerprint cards for a Form 1? I searched around the internet and the ATF website but the only addresses I could find are for Form 4 or FFL License applications.
