The only known cure for CHS is to stop using marijuana. Researchers still aren't sure what causes it.


When she became so uncontrollably sick that she resorted to scalding her skin in a hot bath for the third time in a row, Mrs. X began to wonder if it had something to do with smoking marijuana.

For nearly a decade, the Australian woman had experienced sudden and severe episodes of nausea and vomiting in connection to using the drug. Before that, she'd smoked safely for years with no symptoms. In 2004, a team of emergency room physicians in the country detailed (https://gut.bmj.com/content/53/11/1566) Mrs. X's experience anonymously alongside a handful of similar cases that happened in the same region that year. In nearly all of them, patients described an illness that cropped up suddenly, often after decades of normal marijuana use. Piping-hot baths were their only relief. Although the Australians' report raised some red flags locally, most experts continued to believe that cases like Mrs. X's were rare across the globe. Until a spate of similar reports begin to crop up elsewhere.

Several recent studies from emergency room physicians across Europe and the US are beginning to suggest that CHS could be a lot more common than previously thought.

The latest study was published (http://annals.org/aim/article/doi/10.7326/M18-2809%20) on Monday by a team of clinicians in Colorado, where marijuana was legalized medically in 2009 and recreationally in 2014. In their study, which appeared in the journal Annals of Internal Medicine, the physicians describe more than 2,500 cannabis-related emergency visits to a large public hospital in the state between 2012 and 2016.

Stomach issues like nausea and vomiting were the main driver of the trips, even before psychiatric problems like intoxication and paranoia.

"To see that this was a leading reason for people coming to the ER, that was pretty striking," Andrew Monte (http://www.ucdenver.edu/academics/colleges/medicalschool/departments/EmergencyMedicine/Faculty/Faculty-Anschutz%20Medical%20Campus/Pages/monte.aspx), the new paper's lead author and an associate professor of emergency medicine at ​​​the University of Colorado School of Medicine, told Business Insider.

"We have to do a better job of educating users on the fact that this phenomenon exists," he added.

Two decades ago, the condition was completely unheard of. Then in the early 2000s, a spate of researchers began to describe clusters of stomach-related issues that appeared to stem from heavy and repeated cannabis use. Although they coined a name for the problem - CHS - they still had no idea what was causing it or how to stop it.

More than a decade later in 2017, a team of German ER doctors described (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5360975/) CHS as a "barely known" illness characterized by intense nausea, vomiting, and stomach pain. They still hadn't pinned down a cause, but they did identify a cure: quitting cannabis.

A year later in New York City, clinicians surveyed (https://onlinelibrary.wiley.com/doi/full/10.1111/bcpt.12962) thousands of patients at a large urban public hospital and concluded (https://onlinelibrary.wiley.com/doi/full/10.1111/bcpt.12962) that as many as 3 million Americans could experience something like CHS every year. Again, the only definitive treatment they identified was quitting marijuana completely.

For the latest study (http://annals.org/aim/article/doi/10.7326/M18-2809%20), the Colorado researchers chronicled nearly 10,000 emergency room visits to the University of Colorado Health's Anschutz campus in Aurora, a large public hospital roughly 30 minutes from Denver. Out of all the visits, roughly a quarter (or about 2,500) were related to marijuana.

Of the weed-related visits, 31% were stomach-related - making stomach issues the leading cause of all ER trips for cannabis. And out of the stomach-related cannabis visits, CHS emerged as the single most common adverse event and the main driver of hospital admissions.

Monte's findings are beginning to chip away at the previously held assumption that CHS is uncommon.

"CHS is certainly not very rare," Monte told Business Insider. "We see it absolutely every week in our ER."
Other researchers agree.

The article indicates, but doesn't really say, whether these people are repeat patients or what. We can assume that if people go through this enough to know that only a hot bath helps relieve symptoms, that it's something they've gone through before. I'd be interested to know which of the cited cases are actually people where this has been happening to them every time they use until it gets so bad that they go to the hospital, compared to the number of people there for just a one time thing, like they drank too much and ingested too much weed and now they're puking their guts out.

Most simply will not give up the ganja, and refuse to believe it’s the cause of their intractable retching. It’s real. And yes, I believe it is the newer, higher-potency strains that cause it. There have been discussions at Emergency Medicine conferences describing how the incidence of cyclic vomiting (aka cannabinoid-induced hyperemesis) dramatically increases in areas of legalized high-potency marijuana, as opposed to those areas still relegated to the smuggling of ditch weed.

In your experience, are these people getting to a certain level of "high?" Like, "I can drink 5 beers a night, but every time I drink 13+ I start throwing up." kind of thing? Or more like one day a switch flipped and now any use causes issues? Intolerance that grows over time, so it takes less and less to get sick each time?

My curiosity aside, I kind of feel like this is one of those things that gets blown out of proportion because it's new. Back when I was just old enough to first start drinking I remember people warning me about Redbull Vodka. "Better watch out for those RedBull Vodkas man, I hear if you drink enough of them you can die!" Uhhhhh, isn't that true for any alcohol on the planet, regardless of RedBull? "Oh yeah man, I guess you're right."

The majority are daily users, probably would be considered heavy users. Some claim that they only use a few times a week, no way to know if that’s true. As far as whether it’s blown it of proportion, probably so, considering the number of users out there compared to the 2-3 per day that come through my ER. By that standard, the number of hemorrhagic strokes caused by cocaine is probably blown out of proportion too. I certainly see much more fallout from alcohol and meth than THC. I think that part of the challenge in convincing patients that pot is the cause of their symptoms is the duration of effect. A patient might stop for a couple weeks then still have episodes, so they think it can’t be the causative factor, but it takes months of abstinence to stop having episodic symptoms. Even if they stop long enough to be convinced, one study demonstrated that about 85% will still resume use, and again have symptoms. (That’s off the top of my head - could be a couple points off either way).

I've read a lot on it and it's only associated with very very heavy, long term use. The hot bath thing is also very much associated with it, as for whatever reason, heat is the only avenue to relieve this kind of nausea. (indicating to me that it is some unknown affect/buildup potentially on nerve receptors?) So it does't sound like a "13 beer" type of thing or even an acquired allergy. It sounds more like what happens when you do "waaaaaay too much" of just about anything for too long. After all the guided human evolution of the plant, it's probably the equivalent to smoking a field worth of ancient plant every single day, when you think about it. And no matter how they want to cut it, that's not good for anybody, of any type of thing.

I think prevalence would be higher, but that level of usage isn't exactly common. Plus, people have to be pretty bad off before they go in for that. The irony is some of them just try to smoke more because they think it helps nausea [facepalm]

Weed is known to help with nausea, until it isn't. For example, since weed reduces nausea, people can drink more before they start to feel sick when doing both. But once you get into that red zone of "Shouldn't I be puking now? Guess I'll keep drinking." the puke that eventually comes will be epic.

Anyway, I'm not trying to down play this at all or say it's blown out of proportion. I was just trying to point out that the overall headline of "Do too much of X and you'll be sick!" is kind of like well duh.

I was curious about consumption method as well.

I know some people who have been growing their whole adult lives, and now work for a large dispensary growing for them. As far as I know, there aren't any extreme changes to way that anything is done. This industry is unique because there were a bunch of underground experts just doing their own thing in the shadows for decades, then the laws change now all those home grown experts are able to apply their knowledge at scale. Probably the only part of that industry with an actual steep learning curve. I'm not sure the hybrid strains really mean anything the way most people think they do. All that ditch weed in the past was likely already hybrid strains because people had less resources to try and specialize their home grow. I think that's an interesting question and now I'm going to go ask the guys I know that now grow for a living what their take is.

In many ways, (and this will be unpopular) I do feel that SMOKING it is the modern equivalent to 1940s Tobacco. Largely unstudied, generationally popular, and pushed left and right as "healthy" for the user, akin to vitamins or medicine. More and more, people are an outsider if they don't smoke. Sound historically familiar? I don't think it will have the exact same path as Tobacco, no... And it has some medicinal purposes, but regular recreational use does have significant downsides imho, but we are at stage in society that occludes objective view from either camp, and the studies aren't conducted at this point. That said, you know the result isn't "it's like vitamin c!"

You and I are of an age where there is no longer any peer pressure to do jack squat. Everybody is a homebody with too much on their plate and "don't give a shit" syndrome. It's always different when we're impressionable. I do have family that came out of high school recently (nieces, nephews, etc.) and the pressure is substantially higher, at least in conversation. Peer pressure is perhaps a misnomer, nobody is cornering them calling them a "pussy", but if you don't actively join the conversion in support of weed, it will cost you socially. When you and I were in school, there was camps and opinions in both directions, and that's really changed. So, it's not direct peer pressure, but the societal pressure in general I think is leading a lot more to smoke in their youth, which is the worst time for someone to start toking up. Informally, I'm finding very few of the latest-grads that do not regularly consume MJ, and that's also been a shift. Normally they are homeschooled children, or children of strict upbringing. Any regular kid in public school though? I'd take a $5 bet on each one, sight unseen, that they use MJ. I'll win some, lose some, but I will come ahead in the long run.

I will, I believe homeschool our children when it comes down to it. Public is a gamble, but when it comes down to it, the friends your children make have almost more influence on the direction their lives take than you ever could. And that really is my #1 concern.

And the kids I see in the public school system anymore? Holy hell.

The odds of your own child making friends with decent kids that will "raise them up" is freaking unlikely. At least in this region. The "stock" of decent kids with a future in the school system is miserable. On top of that, ours are well beyond their age in education, so I'd be shooting their opportunities in the foot if I don't let them continue on their path.

IMO it's due to those use to smoking, now overdoing what they're ingesting. Thinking what they smoked was nothing when compared to an edible, potency wise. Possibly an allergic reaction (again) due to potency of edibles, based on how much THC is in a cookie.

I really think it’s a legit issue, for the small percentage of users who get it. It’s weird - many folks can use ABSURD amounts, all day every day, and have no real appreciable negative impacts, health or otherwise. Heck, I think we all have friends who use heavily and are functional, productive members of society. I suspect that, just like some people get cirrhosis from chronically drinking less than most of us, or are missing the gene that allows metabolism of alcohol and get drunk on tiny amounts of booze, THC-induced hyperemesis is real, although doesn’t affect all users equally. The similarities amongst those afflicted by it are surprising, and not just the Sponge Bob pajama pants. Even without knowing anything about the patient, ER staff of any tenure immediately recognize (from down the hall) a particular quality to the noise, vocalization, volume, frequency, and some indescribable sort of emotion wrapped up in the retching of such a patient. And this subset of patients does not respond to the usual meds we give to most nauseous people, but they do typically respond to another “cocktail” of meds, pretty reliably. I do believe that some conditions are “invented”, usually by big pharma, just so they can provide a treatment, but there’s not much for them to gain in this particular issue.

These aren’t the pukers, but are having a bad time, and typically aren’t safe to be managing their own lives for a few hours. Reassurance and time, sometimes a little anxiety medication. The cyclic vomiters usually need much more time and medication, but usually get to go home. Occasionally they need admission to the hospital if their electrolytes are dangerously out of whack, or if we just can’t get their symptoms controlled.

Genetics argument has some merit. 6% of the tested population has a gene that effectively reduces the effects of MJ on the individual substantially. That is sourcing from the actual SNP study, unbiased,reflected in promethese.com (sp) Only reason I know of it is my wife has it. So we know genes have a role in how well a person tolerates? Metabolizes? Etc... Stands to reason a SNP varient has a yet-unknown side effect with the opposite result. Or perhaps even, that 6% comes with a cost. It would be interesting to run all those affected through a special DB query to see what SNPs they all mostly share, if any...

I had a coworker who ran a grow shop on the side, even helped him out with some brick and mortar type stuff at his shop. I can tell you that there?s an obscene amount of chemicals and ?miracle grow? type crap that?s marketed to growers for use from the time they plant seeds till the time they harvest and process the plant. There?s a huge difference between ?organic? pot, the stuff that?s regulated and sold through dispenceraies (this stuff is apparently allowed to have a decent amount of chemicals used to grow it) and stuff that is grown commercially for the black market. I?m not saying that this syndrome isn?t caused by people consuming waaaay too much pot, but there?s got to be a good chance that the chemicals being used could be contributing to stoners getting sick.

That would be reflective of a sub-standard healthcare provider, certainly not the majority in my experience.

Pot heads have ruined Colorado so if some of them get violently ill I guess I'll just have to deal with the schadenfreude.

A coworker was like that. Had a client walk in, who without a doubt smoked one before leaving his vehicle. She was across the waiting room and POOF!. Had us grab her zrytec? and epi pen (last resort) from her room.
Never saw someones face blow up like that and eyes close that fast from a slight hint of weed in the air. She had said previously she didn't party or go to parties where they might be smoking weed, finally found out she was serious.

When you use up all the tokes, it?s useless..... And you know them travelers, they toke all day & night, so 30 tokes will be used up quickly. Therefore, there will be no more 30 toke Magazines shortly.
