I got my SMG 45 about a year ago and have put a ballpark guess of 500 rounds through it. First impressions were awful, constant FTF and FTE, couldn't get the thing to shoot well at all. After calling LWRC I had to send it back to them. I need to call out that their support was awesome! I got an RMA and had the firearm back in just a few days. After getting it back the SMG 45 was awesome, no issues and tons of fun. It worked well both suppressed and non suppressed. The issues according to LWRC was caused by the magazines.

Now for the big problem:
I had the SMG 45 out yesterday and it was acting "funky" where I was getting FTF issues again. After a a few tries and changing to a different brand of ammo I suddenly got a startling slam fire. Had everything pointed in a safe direction but still unsettling when that happens. After this I looked into the action and saw something unexpected, there is a part floating around. After breaking the whole thing down I noticed the bolt had totally BROKEN. For what this firearm costs I am not pleased one bit. I'm going to call LWRC and will post an update with their response.

Can confirm.
When slam fire happened, we both looked at the hole in the mud (downrange), then a each other, WTF.

My buddies had FTF and trigger problems. He sent it in and they squared him away.

Update: LWRC is having me ship them the SMG45 on an RMA. I've been told there is a wait of two weeks to get the parts that they need for the work. All work and shipping is going to be on their dime at no cost to me beyond my time. The representative has been awesome, was easy to contact and responds to questions quickly. As of now I've gotten no answers to the "why" questions, they'll be able to tell me more once they have things in hand. I'll update again in the 2-3 weeks I assume it'll take to get the firearm back home.

Heard the accuracy of the SMG has been garbage from some other reviews. What’s your take on the accuracy of yours?
I’ve been wanting one for quite some time but haven’t heard a die hard positive review yet.

When it actually works I have no accuracy issues. A buddy has one and before sending it back to lwrc it would be throwing the rounds all over the place.

Update: I still haven't gotten mine back from lwrc yet. They're apparently waiting on parts and testing parts they do have. They claim to have discovered a trigger slap issue too. I'll be glad to get it back and finally working but would not recommend buying one after all the issues.

update: was just told that this "went to the range for testing today", they have "lots" of them in for service right now
they claim I should get it back this week or early next week
no answers to my "what was wrong" questions

it's been 80 days since this arrived at LWRC and I was told "two weeks"

I want to recommend DO NOT BUY AN LWRC SMG45

Everyone I've spoken to has been responsive and nice but I think this design has major issues and should not have been sold in the first place.

I finally got my SMG 45 back today. Took quite a while. Going to take it to the range soon for a test. I can post a list later but they replaced a bunch of parts.

Took this out to the range after being fixed. So far things are working great. Not a single issue after testing 200 rounds. Worked well both suppressed and not. Accuracy was about what you'd expect from a .45. Could hit steel at 100 yards pretty easily. So far so good. I still don't have faith it'll be okay in the long run. Going to Form 1 it anyway and take that chance.
