A cleaning patch. Heck of a reminder to make sure your bore is clear. I saw pics one time of a lever gun that had been fired with a plug of mud in the barrel. About 6" of the end of the barrel looked like a banana peel. Yikes!
My bet here is that a CATM troop didn't inspect the gun properly after the previous person qualifying had cleaned it.
Thats one catastrophic failure.

Wow, one would think with the hunk of metal (bullet) and the pressures behind it that a simple piece of cleaning patch or mud would be shot out.... Just goes to show ya never know...

Excellent reminder.
With the receiver blown apart like that I'm quite curious what condition the shooter is in.
