Super easy to do if you get the vehicle specific brackets.

My buddy put in a B&W flip ball, for his goose neck. I talked him into the B&W 5th wheel adapter and I can say it is the slickest thing ever. Mostly because it removes and you get to use the whole bed without any rails in the way. Not the cheapest option but very cool. And B&W is a great company.
I have a friend with one as well. Solid setup.
After repeated warnings NOT to use a 5th-gooseneck adapter on his (ranch setup) rigs and a near death experience when it ripped apart (hey, I told you so...), he did the right thing.
P.S. What is it about the "hydro-formed" rails and Dodge saying not to weld on the frame?
