Anyone done this with a switch in the cab so you can turn it off if you want?

Not trying to be a smart ass, but I can seldom see out the rear view mirror of my company truck. Not a big deal to use the side mirrors when backing up. Best bet is to utilize "pull through" parking whenever possible. In other words, try avoid having to back up at all.
As far as a back up siren you can shut off, I would wire the siren in at the back of the vehicle an run the positive wire off a backup light and run the ground wire into the cab and to a toggle switch an then to ground. In other words, use the toggle switch to turn the ground on or off as needed.
As far as how to get the single wire from under the truck into the cab, it's kind of hard to say without seeing it. There is probably a rubber grommet somewhere on the bottom of the cab or maybe the firewall (that's the "bulkhead" for you GM guys. GM doesn't like to use the word "fire" when talking about its vehicles.) that you could easily poke one wire through with no problems.
