I think we're behind the 8-ball for infectious diseases. It'll be some normal bacteria that really causes problems. A regular old enteric with a KPC or something will spread. Maybe a nasty Gram (+). We just abuse the hell out of antibiotics. I think the era is going to end in my lifetime or shortly after.

TB would also be my base if I were developing a bioweapon, and I know the KGB were working on this decades ago. MDR (or XDR) takes longer than 30 days depedning….but it can be extremely virulent.

You don't need any special joystick or whatever - it is all decision making. You pick your "bug" type and what kinds of properties you want to give it and civilization "fights back."
I actually used to make my ID elective students play it as a lesson.
