I filled out one of these for one of my employees once, a young kid that just did'nt get the whole work thing. He laughed and became a better employee after that because he realized that I had a sence of humor. Go figure.

While I was on duty I was teasing a doctor at our hospital and he gave me a prescription for vagisil, of course I said, "you used to be a cop or in the military, weren't you?" He laughed and said, "both."
