My first four years, once a year I'd bring all our production to a halt for at least a couple hours by making a stupid error (like the time I updated every record *except* the one I wanted, because I used a minus sign, not equals). And one year, one of our DBAs accidentally deleted our entire primary database (he was a superadmin, had the database highlighted in our software, accidentally hit delete, software said "lolk" and did it right there), and since our backup system was (we learned) flawed, we were down entirely for 3 days, and then at reduced capacity for another couple of weeks.
