1 - over tightening it
2 - remove and taper the pin / slight bevel on edge. The cap may be hanging up on the primer / primer pocket.
3 - when in doubt there's theReloading forum
where all your answers can be questioned.

5 - information regarding what dies, press, brass, primers, powder, bullet,. when inquiring about a reloading issue helps.
6 - updated for 5 since i see you mentioned the die. The decapping pin is suppose to "float" for lack of a better term. This allows the pin to be pushed up vs. breaking off, bending, damaging the die.

HB, there's gonna be a learning curve that I think all of us have to get through. The kid broke a lyman die trying to unstick a 223 case.

Amen to that. I learned by reading, asking questions, joining in on multiple reloading forums and OOPS. Catching the OOPS is hard, unless you are aware what is or is not happening and why.
Right now it's a static electricity issue. Low humidity has powder sticking all over the place. Now to find a moisture % that works , yet not too much for metal related issues.

I think I just have a poor understanding of how this is supposed to work. If I lower the tension on the decapping rod, then it just gets pulled out and stuck inside the case every time.

on a lighter note, my dog can now close drawers.
