You should always recheck torque about 100 miles after rotation. Sucks for sure though. Glad nothing worse happened.

I had a rear tire come off my old dodge pick up goin 80 on the highway. Truck went sideways almost immediately and put the front end into the wall of an overpass. Deffinately a pucker moment.

Glad everything turned out ok. I have been through some DW at about 50 mph in my 06 Wrangler and I puckered up pretty tight but I can't imagine watching the wheel go flying off.

That wear on the posts is from the tire wobbling back and forth with loose lug nuts on. Always tighten to the torque specs and never use power tools.

I'm pretty meticulous when I put lug nuts on. I do star pattern, use an impact wrench then check them with a breaker bar or torque wrench.

Herein lies your problem me thinks. If you tighten the lug nuts as described the likelyhood of over torquing them is very real. That will lead to the studs stretching. Stretched they lose their elasticity and will not hold tension on the nut and can allow them to loosen.
And or the threads in the nut begin to strip and eventually fail.
Over torqued fasteners cause just as many issues as loose fasteners.

Glad your OK, I lost the right rear duals on a 1 ton SB 25 @ 17th. Watched them pass me like I was stopped, bounced over the rail into northbound and didnt hit a thing.

There's a man in Sedalia that owns the only known surviving prototype of the Davis 3-wheeled Jeep. I've got to get my ass over there this summer to meet him and get some photos of it.
