Small first aid kits usually equate to a boo-boo kit. Kerlix and / or an Israeli bandage is always a good idea for more serious issues. Things that have multiple uses always seem to get the nod in my bag. A SAM splint, a bandana, chap stick, wool scarf or extra socks would be a few good examples.
Another thing I like for the aid kit is petroleum gauze.
Aside from the first aid uses, you can always cut a slit on one of the faces, pull just a little bit out and use it as a candle. A good leatherman type tool is always worth it's weight in the pack.
I always keep a new bic lighter and wrap it with a bunch of duct tape (or wrap duct tape around a water bottle). Zip ties are light and handy for attaching things to the outside of your pack (among a zillion other uses). No reason not to get an emergency blanket. Very light and small, but very effective.
Lots of good info in this thread. Lots of the things mentioned can be improvised if you need them too (like paracord - you can make darn good cordage from yucca plants, wal-mart bags, et cetera). But it's easier to just have 100 feet of 550 cord on you. There are dozens of ways to start a fire, but that bic lighter is the easiest and pretty darn dependable (if it's cold, put it in your pocket for a minute to warm it up).

cheaper just to do pj cotton balls than the med ones which are mostly useless for med stuff
Just illustrating a point. Like I said, a lot of stuff can be improvised. I had a guy give me a bunch of this stuff. Your budget / your bag...
