No, you don't have to register your Trust anywhere with anyone.
When you submit your Form 4 application, you should submit a copy of the main body of the Trust itself along with Schedule A. If this is the first item you're putting in the Trust, then the only thing that should appear on your Schedule A is the Ten Dollar Bill. Once your Form 4 is approved, you should then revise the Schedule A to include the newly-approved item AND the original Ten Dollar Bill. Keep the revised Schedule A with your original Trust paperwork. It's also advisable to keep a copy of the Trust and the Schedule A physically located with each NFA item held by the Trust.
Another thing to keep in mind is that when you sign the Form 4, you should make sure to indicate that you are signing as Trustee for the Trust and not individually. I include a sample Form 4 for my clients that shows a proper signature line. You should include "[your signature], as Trustee" on the signature line.
