Dumb question - when buying one does someone pay for the suppressor first and wait or pay once the form is returned many moons later?

I bought it first then went back later to do the paperwork and pay the tax. They are holding onto it until the paperwork comes back.

You pay Dealer for the suppressor, file the Form 4 and pay the $200 Tax to the ATF when you send in the paperwork. The only thing you leave with is a purchase receipt for your suppressor.
