I had one that I assumed as you and filled out all of the questions on the form 4, was kicked back until I sent in the responsible person form for myself. I had no other responsible persons involved, but that is how they wanted my form 4 done.
Now I'm even more confused...I thought the 5320.23 was the responsible person form. What form number do I need?
