Oh sweet jeezus! I hate that crap so much! Jeremy Clarkson (of the BBC's Top Gear) puts it best "Americans don't understand that the [left] lane is for passing! You drive slow in the fast lane and drive with your fog lights on when there is no fog! Where are you learning these driving habits?"
Every time someone is in the left lane and NOT passing I turn on my left blinker to kinda hint for them to get over, if they don't move I flash my headlights. I hate how people drive slow in the fast lane... worst place for this ever is on I-70 between Golden and Chief Hosa headed west- mainly because it's uphill and truckers are a**holes who want to try to pass while in low gear going up the mountain so everyone gets over into the left to try to pass them doing 55! I have to admit I wish my car had some RPG's under the hood for these roadhogs. The law in Colorado is still pretty new and has no points so there's not much motivation to enforce it. I drive I-25 every day and I've never seen anyone pulled over for this.
I grew up back east where is was a 1 point violation and was seriously enforced on the major highways so nobody did it. It really kept traffic moving well.

Almost all the time it happens, I flash my high beams at them...never get a response...gonna have to start using the horn.

I use the left lane a lot because it's usually substantially smoother than the right. But I don't camp there, if I see someone coming behind me I always move over. And I'm usually going quite a bit faster than the speed limit. I never run in the left lane under the speed limit, people will run yer ass over.
I commute daily from Parker to Downtown Denver on I-25, and nearly always run in the left lane because I'm expressing it from Lincoln to Auraria Parkway. But I'm running 75-80 too. Staying over in the right lanes is crazy because of all the traffic merging on, exiting off, and changing back and forth. Much safer in the left lane, but I still move over if someone's coming up behind me going faster than I am.
I drive everyday almost all day, about 150 miles a day for work in my own vehicle(outside sales). I see these morons EVERYDAY. Man it chaps my ass.
Someone mentioned about back east, I'm for Illinios, and IL and IN and WI have a "no semi's in the left lane" law...period, end of story. If you are a semi, you are not allowed in the left lane. I wish they would pass a "no moronic spanish women in the left lane" law...they are not the only ones, there are morons of of every race creed and nationality in the left lane going slow.
