Herein lies your problem me thinks. If you tighten the lug nuts as described the likelyhood of over torquing them is very real. That will lead to the studs stretching. Stretched they lose their elasticity and will not hold tension on the nut and can allow them to loosen.
And or the threads in the nut begin to strip and eventually fail.
Over torqued fasteners cause just as many issues as loose fasteners.
Glad your OK, I lost the right rear duals on a 1 ton SB 25 @ 17th. Watched them pass me like I was stopped, bounced over the rail into northbound and didnt hit a thing.
