I submitted the paperwork for my suppressor to the BATFE and the check was cashed in August. I would think that it should be sitting there, I have gotten paperwork back for items I ordered after those dates. (the other items order from other area shops). I'm going to try the phone numbers Sam dropped in here on Monday.
Have you called the ATF to ask about the status of your form 4?

I pre-purchased a lower from them way back when they were first offered. I was hesitant and I went against my better judgment but I did it anyway. It took over a year but I did get my lower and a matching upper. I am one of the lucky ones.
